Townhill's on-line services allows you to book Doctors appointments on the day and up to three weeks in advance, order your repeat prescriptions have access to view certain elements of your medical record. You need to complete a registration form to register for our on-line services. Your on-line account services will not be activated until we have seen the required proof of identity.
Here's how to register
  1. Download the registration form by clicking on the button below.

  1. You also need to download a copy of the Patient Access Information Leaflet here.
  2. With the completed registration form and proof of ID and address (list of acceptable documents are found with the registration form), come to the surgery and hand into the receptionist who will then print off a token letter for you.
  3. The token letter contains full instructions on how to complete the registration process.
  4. If you encounter any problems please call the surgery.


NOTE:  To register for Online Services you need to be 16 years or over.
You will need to present at Reception in order to register as we are required to  verify the documentation.  



We are pleased to now offer Proxy Access. This means that you can book appointments and order repeat prescriptions for your child/relative/person you care for, with their consent. For children under 11, please inform the Receptionist. For anyone who is 11 or over, verbal or written consent must be given by the patient to the Receptionist to enable us to set up Proxy Access. Please enquire at Reception if you have any queries about this.


Patient On-line Access Statement
All patients (over 16 with a separate email address) can register for Patient Access On-line Services and given access electronically to certain functions. These currently are:
  • Electronic booking of appointments
  • On-line requests for repeat prescriptions
  • View and print certain elements of their clinical record
The practice also already meets the Governments stated requirement of a daily upload to the Summary Care Record and the use of GP2GP to electronically transfer records when patients register or de-register at the practice.
